If you have ANY of the following symptoms:

Fever
Cough
Sore Throat
New onset Shortness of Breath

And/Or have been exposed to anyone with the Coronavirus, please reschedule your appointment for no sooner than 14 days and follow up with your PCP.

At your scheduled appointment you will be screened before checking in and ALL patient family members and drivers are being asked to wait in the car unless the patient needs assistance.

Notice of Access to Patient Health Information:

We support our patients having access to their health information through the Patient Portal. Beyond the Patient Portal, we can enable certain mobile apps that meet requisite technical specification and security requirements to provide you access to your health information in a safe and secure way. Currently no mobile apps have been configured to connect to our system. Our portal will be updated if any additional apps become available for you to use to access your health information.
